AFFIDAVIT OF GUARDIANSHIP

I, MYLIN BAÑO QUIAPE, of legal age, Filipino and resident of Kaban,


Brgy. Kaypian, City of San Jose del Monte, Bulacan, after having been duly
sworn to in accordance with law, do hereby depose and say that:

1. That I am the auntie of the minor KHALED Q. ALMUNEEFI who is


under my custody and guardianship;
2. That I am competent to certify in behalf of his parents for whatever
legal purpose;
3. That I am not an imbecile, insane, a vagrant or vicious person or a
habitual drunkard or a habitual criminal, and have not abandoned,
neglected or refused to support said minor or caused him to commit
offenses against the law;
4. That I am executing this Affidavit to attest to the truth of all the
foregoing facts and for whatever legal purpose it may serve.
