Dear Ms. Flores, Re: Request for Reconsideration, ROSEMARIE P. TATOY I am writing this letter to let you know that I and my family are pleased to have Starcare as our HMO provider. We have been members since 2007 and we are happy that you continue to provide for our medical and healthcare needs. Hence, we are extending these benefits to our youngest daughter, Rosemarie P. Tatoy. We thank you for the approval of her application and though we accept the provisions of her healthcare coverage, we kindly request to reconsider the Annual Physical Examination (APE) coverage under her program. We believe that this procedure is necessary to determine her health condition. Because this provides an opportunity: to know more about her medical status; early detection and prevention of any disease; and of course to teach her good health practices in line with your belief that Health is wealth. Note: Please see No. 8 provision on the attached billing.
