AFFIDAVIT OF LOSS I, Jessalyn I.

Basco, of legal age, single and residing at San Roque, San Pedro, Laguna, after having duly sworn, do hereby depose and state that: 1. I am the holder of a SIM card no. 0927-656-2802 at Globe Telecom; 2. A few weeks ago, My cell phone was stolen/lost while Im riding at the jeepney going home from school; 3. I took all necessary efforts to be able to find my stolen/lost cell phone, but the same was proven futile; and 4. I am executing this affidavit to attest to the truthfulness of the foregoing statements and for whatever legal purpose/s it may serve. IN WITNESS WHEREOF, I have hereunto set my hands this 27th day of July 2012 at San Pedro, Laguna
