MEMORANDUM OF AGREEMENT This Memorandum of Agreement is entered into between __________________ with business address at _________________________, and First

Asia Institute of Technology & Humanities (FAITH) , with business address at #2 Pres. Laurel Highway, City of Tanauan, Batangas. This Memorandum of Agreement signifies the affiliation of two parties in providing the students handson experiences in their chosen fields. Terms and Conditions: A. Students 1. FAITH shall inform the number of students particularly BS Accountancy / BS Business Administration who are scheduled to have an On-the-Job Training (OJT). 2. FAITH shall schedule the availability of students that will be sent to __________________ based on their free time as per students curriculum. B. Training 3. __________________ shall provide the hands-on experience to FAITH students by assigning them to areas that will enrich their learning experience. 4. __________________ shall determine the department / office to which students will be assigned for their training relative to the course of the student. 5. __________________ shall provide 300 hours (_______, 2013 to _______, 2013 ) of work experience to FAITH students as required by CHED. The same maybe extended, however, depending upon the supplementary agreement among the parties herein. 6. __________________ may provide the students a training allowance and other benefits, if deemed necessary. C. Monitoring and Evaluation 7. __________________ shall provide an evaluation of the students after completing the required number of hours. 8. __________________ shall inform FAITH of any problems regarding the students during the OJT period. 9. __________________ has the right to refuse acceptance of students for OJT when no request is received from any of its department / offices. 10. __________________ shall issue a certificate of training after completion of the required number of hours. 11. __________________ agrees to make a report to the OJT Program Coordinator during his /her visit to the place where the student is assigned.
