Scenario
Hillary Jackson is a hard-working registered nurse (RN) who consistently provides excellent
quality care to her assigned clients. She always helps others when her work is completed and has
good working relationships with physicians, families, and other health care workers.
One area that Nurse Jackson despises is charting. She charts in a cursory objective style but does
not always document everything that is needed. Rarely does she contribute to the nursing care
plan, which necessitates others completing this work.
Nurse Jackson is active in the Oncology Nursing Society and regularly attends continuing
educational offerings. She has joined the unit-based quality improvement committee and
contributes valuable information.
Directions
The following is a form that you must complete as Nurse Jacksons peer evaluator. Remember to
make the notations as objective as possible. Descriptions of each category are listed below the
client behavior and Likert scale. A Likert scale is used to rank her performance, with a range of 1
for outstanding to 5 for unsatisfactory.
When making comments about Nurse Jacksons performance, use specific measurable objective
data. For example, a comment in the professional development section might read as follows:
Nurse Jackson has been a member of the unit-based quality improvement committee for 4
months and attends regularly. She has brought many ideas for improving processes on the unit,
such as walking rounds instead of report, flexible hours for employees, and the need to develop
procedures for new oncological treatments.

Comments: Ms. Jackson would receive a 1 for Client Care.
She provides her patients with excellent care, she was also on time treatments and scheduled
medications.
Client Teaching

Comments: Ms. Jackson receives a 4 for Client Teaching.
She is very incomplete with her charting, she does not include all of the information that is
needed. Which will affects the care the patient receive on the shift that follows her.
Committee Participation

Comments: Ms. Jackson receives a 1 for Committee Participation.
She is involved in the Oncology Society and attends meetings regularly and actively contributes.
Charting

Comments: Ms. Jackson receives a 5.
Her charting has been incomplete and she does not contribute to the nursing care plan, which
necessitates others completing this work.
Nursing Care Plans

Initiates care plans for newly admitted clients during the shift of admission and updates and
modifies nursing care plans.

Dresses professionally, participates in professional organization activities, attends continuing
