(A) Policy Statement
Admission of patients shall be performed in a timely manner with efficiency and reflect the Philosophy of
Nursing at UTMC. Nursing personnel shall encourage open communication with both patients and family
members whereinformation is given and received.
(B) Purpose of Policy
To provide standardization of services and initiate a climate of mutual support and appreciation among patient,
family, and staff.
(C) Procedure
Patient's condition may warrant changes in sequence. (*May be completed and documented by Licensed
Practical Nurse (LPN) or Nursing Assistant (NA). A Registered Nurse (RN) must complete remaining
procedure.)

Orient patient to the nursing unit.
Initiate/review and complete Admission data with history with the patient/significant other including
mutual discharge goals to be used as a guide for patient care. Admission data with history must be
completed within 24 hours of admission.

Identify location of the guide to patient services book for the patient/significant others, which includes
information about safety, patient's rights and responsibilities, advanced directives, etc. Review and offer
clarification/additional information or contact appropriate resource personnel to assist patient/significant
other.

The Clerical Specialist will:
a.
Check to ensure that the patient is wearing an identification band.
b.
Follow procedure for physician notification.
c.
Verify admission order (In-patient vs. observation) with registration designation. If not the same,
notifies admitting office.
d.
Mark room and bed number and patient's name on the identification label with color-coded tape.
e.
Use identification labels on paper forms within the Medical Record.
f.
Identify outside of chart, using white card for the patient's name and the color-coded tape to identify
service and room number.
g.
Initiate Kardex (on unit where used). Include allergies, referring agency, diagnosis, condition,
patient and/or significant other identification, etc.
