* Please make checks to Cathy Taylor
* Please apply sunblock at least 30 min before lessons start so as not to upset the pool chemistry.
* Make-up lessons are only offered in cases of prolonged or extreme sickness, accidents, or death in the
family. These lessons are arranged with the instructors at their convenience. Friday make-ups will be held on
the Friday immediately following a lesson canceled because of bad weather.
* Please supervise children who are not swimming and do not allow them to play on the swing set/sandbox area,
trampoline, or covered hot tub. Please do not allow them to climb trees. You are welcome to use the Little
Tykes play area.
* Children may not enter the water until the instructor begins the class.
* Please have children use the restroom before coming to swimming lessons.
* Please be supportive of one another and remember to have fun! .

Release Form
I hereby release all members of the Ries family and employees/contractors of Cathy’s School for ‘lil Fish from
any and all liability of any nature for any and all injury or damage (including death) to me or my minor
child(ren) and other persons observing lessons as a result of my participation in swimming lessons, even if
caused by the negligence of any of the releasees named above, or their employees, agents, servants or
assigns.
I have read this release of liability agreement. I understand that by signing this document, I am waiving
valuable legal rights, including any and all rights I may have against the owner, the operator named
above, or their employees, agents, servants or assigns.
