AFFIDAVIT OF UNDERTAKING
I, ________________ , of legal age, Filipino, and a resident of____________ ,
under oath, do hereby depose and state:
1. That I am an applicant/ enrollee of College of Law, Western
Mindanao State University;
2. That I am a graduate of Bacherlor of Arts major in Political
Science, Western Mindanao State University;

That considering the lack of time and unavailability of my


partners in thesis, the result is inconvenience and difficulty in the
processing of my clearance/ thesis approval which is a
prerequisite for the release of TOR;

4. That a prerequisite for this years admission to College of Law


strictly requires the compliance of Law Student Admission Test;
5. That I was not able to comply the same, due to the absence of
my Transcript of Record;
6. That I failed to present said document, due to the fact that my
TOR is still in the process, and it will take few more days after the
scheduled admission test for it to be presented;
7. That I wish to take the Law Student Admission Test which is
scheduled on May 22, 2016;

That this action does not mean to delay nor to violate the
procedure of enrolment/process or law school admission, but a
means for substantial consideration of this matter for this
situation is beyond my control;

That, I undertake to take the Law Student Admission Test and


commit myself to produce my Transcript of Records, immediately
thereafter it be issued to me;
10. That I am executing this affidavit to attest to the veracity of
the foregoing facts and for all legal intent and purposes as this
may serve under the law.
