Under and by virtue of the authority prescibred by Sec. 3, Rule
112 of the Rules of Court, you are hereby notified that a Complaint has
been filed against you by the above-named Complainant. A copy of
said Complaint together with other supporting papers/affidavits is
herewith served upon you with the instruction that you submit your
ANSWER or COUNTER-AFFIDAVIT and supporting documents to the
undersigned investigating prosecutor within TEN (10) DAYS from
receipt thereof. Other evidence submitted to this Office in connection
with the case is available for examination by you and your counsel.
Failure on your part to comply with the answer and other
supporting evidence within the prescribed shall be considered a waiver
and a case submitted for resolution.
