Nursing Care for Fatigue Management

The patient reported feeling very tired and weak. An assessment found fatigue, weakness, disinterest and normal vital signs. The plan was to have the patient rate their fatigue, help develop a fatigue management plan, schedule rest periods and allow the patient to increase activity as tolerated. Nursing interventions included assisting with needs, encouraging activity and nutrition. After 3 hours the patient reported an improved sense of energy.
