I am / We are aware that joining the AMACC Intramurals16 Cheer dance Competition
may result in personal injury. While the organizers will take all possible precautions, I/we
recognize that accidents and injuries may still occur. By signing this waiver form, I/we
assume all risks and the consequences of those risks.
I/we will not hold AMA Computer College- Cotabato together with their employees liable
for any personal injury or any personal property damage that may occur on the premises
before, during, or after the event. Furthermore, by signing this Waiver and Release Form,
I/we assure that I/we have read, and agree to abide by, all the rules and regulations
covering AMACC Intramurals16 Cheer dance Competition and take full responsibility for
following these rules and regulations, including the consequences of my failure to do so.
