AFFIDAVIT OF LOSS
I, MA. CRISTINA DAGAMI y COLES, of legal age, married, member of
Philippine National Police and currently the Chief of Police of Dolores
Municipal Police Station, Dolores, Eastern Samar, after having been duly
sworn to in accordance with law, do hereby depose and say THAT:
1. That I am the Team Leader of the joint Dolores Municipal Police
Station and PDEA RO8 Buy-bust operation which resulted to the
arrest of DANILO ACASO y ABIQUE, 46 years old, married,
resident of Sitio Sandalogdog, Barangay Dapdap, Dolores, Eastern
Samar on June 9, 2016 at around 7:15 oclock in the evening at the
aforementioned barangay;
