2) Root Cause and Action Taken

Visual inspection of deburred parts was insufficient as burrs were too small to see, potentially causing product leaks. To address this, parts will now be sorted using a scope under a new standard operating procedure, as visual inspection places undue strain on operators with eye problems and does not ensure complete deburring. The new 10x scope inspection and formalized procedures will help catch any small burrs to prevent product defects.

Possiblitity Root cause

Current checking method is using visual method. Not using any equipment such as
scope. The burr is to small to be see by naked eye. This method might cause miss
deburring and product leak out.

Action Taken
Product need to be sorting using scope because burr is too small. It will be very
difficult for the operator who had eye problems. The small burr must be deburring
using 10X scope SOP manual will be drafted to ensure that the operator in
accordance with the right working procedure.
