Class - ACE Inhibitor: Captopril

Captopril is an ACE inhibitor used to treat hypertension, congestive heart failure, and diabetic nephropathy. It works by inhibiting the angiotensin converting enzyme, preventing the conversion of angiotensin I to angiotensin II and causing vasodilation and increased cardiac output. Common side effects include hypotension, cough, angioedema, and hyperkalemia. It is well absorbed orally, 30% protein bound, and eliminated renally with a half-life of 2 hours. Studies show captopril decreases mortality from congestive heart failure and improves symptoms and exercise capacity.

Mechanism of Action

- an inhibitor of angiotensin I-converting enzyme (ACE), the enzyme responsible


for the conversion of angiotensin I to angiotensin II -> vasodiation, increased Q,
myocardial remodeling.

Pharmaceutics

- tablet
- 12.5 to 50mg

Dose

- PO: 12.5 to 50mg Q 6hrly

Indications

- HT
- CHF
- diabetic nephropathy

Adverse Effects

- hypotension
- cough
- angioedema
- anaphylaxis
- cholestasis -> hepatic failure
- hyperkalaemia

PK

Absorption 75% bioavailablity


Distribution 30% protein bound, Vd = 0.8L/kg
Metabolism - hepatic
Elimination renal, t1/2 = 2 hrs

Evidence

-> shown to decrease mortality when used in congestive cardiac failure
