AFFIDAVIT OF CHANGE OF BODY

I, MARIA DAIDEN MATA OCENAR, of legal age, Filipino, married and resident
of Block 80 Lot 4, Phase 4, Mabuhay City, Paliparan, Dasmarias, Cavite, after being
duly sworn to in accordance with law, hereby depose and say that:

1. I am a lawful owner of a motor vehicle which is described as follows:

MODEL : 2011 ENGINE NO.: RMC125TJRB00003850
TYPE :MOTORCYCLE PLATE NO. : 3757 WS
SERIES : BS-125-42

2. In the original certificate of registration (CR) said motorcycle is without


sidecar, I have caused the installation of a side car on said motorcycle due to fact
that I am using it to my small business.

3. I hereby warrant/guarantee and hereby absolved the Land Transportation


Office its personnel and officials of liability whatsoever and hereby take all
responsibilities resultant of this affidavit.

5. I am executing this affidavit to attest to the truth of the foregoing facts


and for all its legal intents, effects, consequences and purposes, including to
request from the proper government authorities for a change of denomination from
without side car to with side car.
