Waiver Team

This document is a waiver form for students participating in a team building activity organized by the Philippine Institute of Chemical Engineers - Junior Chapter Batangas (PIChE-JCB) at Batangas State University from December 20-21, 2014. The waiver form releases the university and establishment from any liability for injuries the student may suffer during the activity. It also makes the student responsible for any damages to property or injuries to third parties that may be caused by the student during the course of the activity. The student and parent/guardian must both sign the waiver form to acknowledge the terms.

third person/s which may be caused by me intentionally or inadvertently while in
