AFFIDAVIT OF DISCREPANCY

I, MARIA DE LA CRUZ MAKILING, of legal age, Filipino, widow of


JUAN MAKILING and a resident of ____________________________,
Philippines, having been duly sworn in accordance with law, hereby
depose and state:

1. That I am the mother of APPLE MAKILING who was born on 1


January 2010 in _______________________ as evidenced by her
National Statistics Office (NS0) Certified Certificate of Live Birth,
a copy of which is herewith attached as Annex A.

2. However in the school records of my daughter Apple, she has


been erroneously using APPLE DE LA CRUZ, which is my
maiden name, instead of APPLE DELACRUZ MAKILING which
is her true full name.

3. APPLE DE LA CRUZ is one and the very same person as


APPLE DELACRUZ MAKILING who is studying at Banahaw
Elementary School at ____________.

4. That I am executing this Affidavit of Discrepancy to attest to the


truth of the foregoing facts and to explain the discrepancy in my
daughters school records as regards to her full name, and to
reconcile the same with her official Certificate of Live Birth
records and for whatever legal purposes this may serve.
