I, DEO ROSALES, of legal age, with address at Brgy. Buri, Palo, Leyte,
after having been sworn to in accordance with law hereby depose and state
that:

1. I am the Municipal Accountant of the Local Government Unit of Palo,


Palo Leyte;
2. I am executing this affidavit upon request of Giles Edgar Daya,
former Administrative Aide I of the Local Government Unit of Palo;
3. Mr. Daya has been appointed as Court Interpreter II of the Municipal
Trial Court In Cities Branch II Tacloban City;
4. Upon the effectivity of his resignation on February 10, 2017 or
subsequent thereto, and having been cleared of all money and
property accountabilities, and until today, Mr. Daya has not received
any gratuity or separation pay in relation to his resignation from the
Local Government Unit of Palo;
5. I herein attest to the truthfulness of the foregoing facts.
