AFFIDAVIT OF DESISTANCE

I, JARDIOLE L. CRESPO , of legal age, _______and a resident of Kabatiol,


Maasim, Sarangani Province, after having duly sworn to in accordance with law
hereby depose and state:

1. That my motorcycle was involved in a vehicular accident transpired at


Kabatiol, Maasim, Sarangani Province on April 29, 2017 driven by
KENNETH M. RICALDE and that of government vehicle issued under the
name of ROSEMARIE MARTINEZ-SAYO, with plate number SGX 664
driven by ROLAN OMILA;

2. That I have already entered an amicable settlement with ROLAN OMILA


and ROSEMARIE M. SAYO, hence, I am no longer interested in filing a
case, either civil or criminal against ROLAN OMILA and ROSEMARIE
M. SAYO nor testify against them in any court or administrative agency,
arising from the said vehicular accident;

3. That I am executing this affidavit in order to attest to the truthfulness of the


foregoing statements.
