JOINT AFFIDAVIT
We, ANDREW LEW LOPEZ ANDRADA and JASON PARIS
MOLANIDA, of legal age, single/ married, Filipino and residents of Roxas
City, after having been duly sworn to in accordance with law, do hereby
depose and state;
1. That we know JAYSON JAY RIVERA OROA for more or less 20
years because we are friends;
2. That he is the biological father of KEISHA GAIL D. OROA,
who was born on June 13, 2009 at Birthing Clinic Placor, Masbate.
3. That after one (1) year from the time, Lilian Batan Dordas give
birth to KEISHA GAIL D. OROA, his biological father Jayson
Jay Rivera Oroa abandoned his family and until now he cannot
be located.
4. That Lilian Batan Dordas exerted effort to locate his whereabouts
but to this date they have not heard anything from him.
5. That we are executing this affidavit to attest to the correctness
and truthfulness of the foregoing stated facts.

I, JOSE RODELEN A. BAES, of legal age, married, Filipino and
resident of San Jose Village, Tiza, Roxas City, Capiz, after having been duly
sworn to in accordance with law, do hereby depose and state;
1. That KEISHA GAIL D. OROA is my granddaughter as she is the
daughter of my niece, Lilian B. Dordas. My named granddaughter
is a minor;
2. I am a Branch Accountant of Banco de Oro. I have been practicing
my profession in Roxas City since February 1988 to the present;
3. My named granddaughter is intending to travel with me to Bangkok,
Thailand on June 27, 2017 to July 3, 2017. I will shoulder all her
expenses relative to our travel to Bangkok, Thailand inclusive of our
airfare and accommodation;
4. I am executing this affidavit to attest to the truth of the above
foregoing facts and whatever purpose and legal intent this may
serve.

I, LILIAN B. DORDAS, of legal age, single, Filipino and resident of
Brgy. Tabuc, Pontevedra, Capiz, after having been duly sworn to in
accordance with law, do hereby depose and state;
1. That I am the biological mother of KEISHA GAIL D. OROA,
minor, 7 years old;
2. That in connection with the intention of JOSE RODELEN A.
BAES, my uncle in visiting Bangkok, Thailand, I am giving my
consent for my daughter KEISHA GAIL D. OROA, to travel
with my uncle JOSE RODELEN A. BAES, who is my daughters
grandfather.
3. My uncle JOSE RODELEN A. BAES raised and took care of my
named daughter since I gave birth to her. I have full trust and
confidence that my daughter is in good hands during the travel
because my uncle has actually raised her well and provided for all
her needs;
4. That this travel to Bangkok, Thailand is actually an opportunity for
my daughter to visit tourists spots and popular landmarks in
Thailand;
5. I am executing this affidavit to attest to the truth of the foregoing
facts for all legal intents and purposes.
