Warm Greetings!

I, Jeryvelle Andres, currently studying nursing in section NUR 141, am formally writing this
letter to inform you of my request for a leave of absence during the first week of classes this
coming 1st semester, from June 19- June 27, as I will be in another country from May 27 -June
27 this year, 2017, to attend important family matters.

I have informed my professors of my current situation. Attached to this letter is the itinerary of
my flight for legitimacy purposes.

Approval of request will be greatly appreciated. Thank you so much for your time and
understanding.
