Ms. Dozier,

I couldnt find an email address for the police chief, so Im addressing a public records request to
you.
WCPO requests copies of records documenting discipline in the Addyston Police Department
since January 1, 2013. Our request includes the following:

- All records documenting discipline of every type, and the records that document all
incidents examined to determine the discipline. None This would include Employee
counseling None, Written reprimand None, Recommendation regarding loss of leave None,
Recommendation to suspend (paper and non-paper suspensions) None, Recommendation to
demote None, and Recommendation to terminate None.

- All records relating to disciplinary matters that document the involvement of the
labor organizations representing APD employees, including; all Grievances, Appeals, Last
Chance Agreements, Reports of Findings of Pre-Disciplinary Hearings, Reports of Findings
of Post-Disciplinary Hearings, all documents provided to the Neutral Administrator for each
Disciplinary Hearing, all documents involved in all appeals of disciplinary action against the
employees, including all reductions of discipline, and the final disciplinary action after an
appeal. None
