1. References:
a. Sinumpaang Salaysay of ( Private Complainant) dated __________ with
attached documents;
b. PNP Memo Circular No. 2016-062 (PNP Comprehensive Internal
Disciplinary Mechanism) dated October 3, 2016; and
c. NAPOLCOM Memorandum Circular No. 2016-002 dated March 9, 2016.

2. This pertains to the administrative complaint filed by (Complainant) against


PO1 ____________, presently assigned at the (Unit) for alleged (Specific Offense
Committed) which constitutes Grave Misconduct.

3. Pursuant to Rule 21, Section 2.C, 3 (r), of NAPOLCOM Memo Circular No.
2016-002, crime punishable under the Revised Penal Code or Special Laws where the
duration of the imposable penalty is imprisonment of not lower than six (6) months and
one (1) day is categorized as Grave Misconduct. As such, it is only proper to refer the
subject matter before your office pursuant to Para 8, sub para 2, letters k, h and i
(Investigation Stage) of the PNP Memo Circular No. 2016-062 which states that:

grave offenses or involving breach of internal discipline and light offenses or less
grave offenses involving violation of RPC and Special Laws shall be taken
cognizance by NHQ. xxx

i) Only referral for PCI on grave offenses involving violation of RPC and
special laws shall be taken cognizance by IAS/RIAS in addition to the authority
provided by R.A. No. 8551.

4. In this connection, forwarded to your Office is the entire administrative case


record of PO1 ____________ for your appropriate action.
