1. Application must be accompanied with:


Four recent passport size photographs of the Applicant.
Two photo copies of applicants and his/her Fathers/Guardians CNIC (copy of passport of
applicant if foreigner)
Declaration from Parents and Sureties/Guardians (on form SUH-2) before the Judicial Magistrate
/ Civil Judge of the area of the residence of the applicant.
Declaration form (SUH-2) in respect of Foreigner Student and nominee of any Government etc.
shall require only to be certified by Director Admission, University of Sindh.

2. Application form is to be filled in duplicate, incomplete application shall not be


considered.
