This is to certify that Maricar C. De Leon, a student of Technological Institute of the Philippines to
undergo overtime in his/her On-the-Job Training/Practicum at Jessie James Tamayo Nuez, Inc. on
August 15, 2017, August 18, 2017 and August 19, 2017.

The schedule would be:

Tuesday: 4:30pm 5:30pm


Friday: 3:00pm 3:15pm
Saturday: 4:30pm 5:30pm

We also hold the Technological Institute of the Philippines free and harmless from any responsibility nor be
held liable whatsoever for any injury, personal or pecuniary, which may happen to him / her and / or caused
by third person within or outside the company premises throughout the duration of the On-the-Job
Training/Practicum.
