Slowdown – Important
Dear Chief Engineers & Engine Team;
We would like to share the following event occurred on board of a 37K tanker.
Short Course of Events
The "fuel cam abnormal" alarm sounded and number 3 unit exhaust temperature started
dropping followed by exhaust gas temperature deviation alarm and subsequently Main Engine
slowdown initiated.
What have caused the Main Engine Slowdown?
The Engine Team investigated and found on local emergency panel that number 3 unit fuel cam
astern led was active and the reversing cylinder actually in astern position. The Engine Controls
transferred from Bridge to ECR. The Main Engine restarted with all units reversing cylinders in
ahead position. The Engine loads slowly increased to half ahead when again the "fuel cam
abnormal" alarm activated. It was observed that number 3 unit reversing cylinder had gone astern
as the cylinder head did not took full position ahead due to malfunction of reverse cylinder
system.
The Corrective Action
The reversing cylinder replaced with a new spare. Main Engine restarted the engine. All
parameters were found normal. Engine controls have transferred back to Bridge Again.
Investigation finding
The reversing cylinder opened for inspection and it was found that the l'packing (item 23 ,part
number 50100-23) on one side of the reversing piston, cut as seen in the below photographs.
The internal condition of the pistons and rubber components was very poor.

Conclusion
Dear Chief Engineers & Engine Team;
The present PMS Job 1-1-11-9 Reversing Air Cylinder contains adequate provision with 5 years
interval to prevent this kind of material failures. Parallel to your on Spare Stocks the overhaul
should be carried out or alternatively the job should be postponed to an agreed time with
Office as we explained in our Letter to you – “Why Post-Pone Management is important”
dated April 1st 2013.
