1. Reference: Order from that office dated December 1 2017 directing the
undersigned to explain in writing for failed to make a return on the warrant of arrest issued
on April 7, 2017.

2. Relatedly, the undersigned PNCO because of having a multiple tasking and


a lot of Patrol and Anti-Illegal Drug operation I forgot to return the said warrant of arrest
which was kept inside my filing cabinet.

3. That the undersigned has no intention not to return the said warrant of arrest
and the same could not be repeated anymore.

4. As prayed for, the undersigned asked forgiveness and be given another


consideration for my failure to return the said documents to that Honorable Court.

of PNP Declared Legal Beneficiaries for Retiring PNP Personnel on given target date.

5. Relatedly, after received the said memo, the undersigned immediately


informed the concern personnel to secure all the needed documents and always
reminded to comply.

3. That the undersigned has no intention to ignore the said memo or violate
any orders/directives coming from higher headquarters.
