AFFIDAVIT OF MARITAL CONSENT

having been duly sworn to in accordance with law hereby depose and state:

1. I have applied for the financing services of INSULAR SAVERS BANK, INC.
with office address at Net Plaza 31 st St., Bonifacio Global city Taguig with the
vehicle Plate No.________ as collateral of this loan.

2. That as part of the documentary requirements, my spouse,


__________________________ and I are being required to sign as co-
mortgagors the documents pertaining to the loan application and financing
transaction;

3. That my spouse, _________________________, cannot possibly sign the


said documents because _________________________;

4. That he/she is aware however of my intent to apply for car financing with
INSULAR SAVERS BANK, INC. and he/she has given his/her permission to
such application;
