Re:- Ms. Annette MacNamara

DOB: 14/06/1939

I am writing to request daily home visit by blue care nurse to provide care and support to this
patient, a 71-year-old single age pensioner.

Ms. MacNamara was admitted to our hospital on 20/05/2012 due to right wrist fractured resulted
by fall while descending stairs.Stitches was performed in her kacerated wounds which will be
remove during her appointment on 31/05/12 at 10:30.As she has no- further complication therefore,
she was discharged on 22/03/2012, However, home assistance was arranged.

Regarding to her social history, due to her limited social contact she is lonely and isolated.However
her niece stella visit to her once in fortnight to help her in shopping and other household works.

Medically, she takes her regular blood pressure medication and sleeping tablets as well as doctor
prescribed panadol 4 hourly 1 grm for pain.

It would be greatly appreciated if you could manage your one of the staff to assist her during
showering and wound drelling. Please note that worker will arrange meals on wheels and
physiotherapy. I would appreciate your care to this patient.
