JOINT AFFIDAVIT OF COHABITATION

We, EDISON O. DANGKEO and KRISTINE CHARIE G.


TOLENTINO, both of legal age, single, Filipino citizens and residents of
Purok 4, Bueno St., Brgy. Aggub, Solano, Nueva Vizcaya, after having
been duly sworn to in accordance with law, depose and say:

1. That we have been living together as common-law spouses since


August 5, 2009 and out of this union, we were blessed with one
(1) child, namely: ZHARIE DIOSEN TOLENTINO who was
born on August 21, 2015 at PLT Hospital, Bascaran, Solano,
Nueva Vizcaya;

2. That since 2009, we having living together as common-law


spouses, or more than five (5) years, without any legal
impediment to marry each other;

3. That we now desire to validate our cohabitation by contracting a


marriage;

4. That we are executing this affidavit in lieu of a marriage license


under Art. 34 Family Code No.209, as amended;

5. That this joint affidavit is executed to attest the foregoing facts.

WITNESS OUR HANDS this 2nd day of August at Bayombong,


Nueva Vizcaya.

corroborate under oath the foregoing affidavit of cohabitation executed by
EDISON O. DANGKEO and KRISTINE CHARIE G. TOLENTINO, who
are known to us personally.
