AFFIDAVIT OF LOSS

We, SATURNINO D. GALVE, JR., and MARIETTA BUTIL GALVE,


Filipinos, of legal age, and residents of Brgy.19, Purok 2, Buhangin, Butuan City after
being duly sworn to in accordance with law hereby depose and say:

1. That sometime in the year 2002 we executed a Joint Affidavit of Legitimation


for our child Holy Grace B. Galve;

2. That sometime in the said same year, our copy of the said Affidavit was lost;

3. That we exerted all efforts to locate the same but could not remember exactly
where we placed it and considered it lost for all legal purposes;

4. That we are executing this Affidavit to attest to the truth of the foregoing
statements that the said Affidavit of Legitimation was lost and never recovered.

FURTHER AFFIANT SAYETH NONE.

IN WITNESS WHEREOF, we have hereunto set our hands this _______th day of
