Dear Sir /Madam,

Re: Mr .Bill O’Riley

I am writing to refer Mr.O’Riley,a 53-year-old single who was admitted to our hospital on
02/09/2009 with the diagnosis of obstructive coronary artery disease. He had coronary artery
biapass grafts on 4 th September.

Mr.O’Riley, who lives alone in own home outside Goondiwindi, and works as fencing contractor and
has only one brother .He is heavy smoker and drinking alcohol around 2 bottle per day .He usually
eats junk food and his body mass Index is high.

During hospitalization, he has recovered well after his opration.The wounds are healing well and he
is walking normally now .He has been advised to quit smoking and to reduce drinking alcohol and
also avoid unhealthy food .He occasionally has visited by a social worker.

He has an appointment for follow up by a local general practitioner Dr.Avril Jensen at 2 pm on


15/09/09. He needs to be visited by a local physiotherapist for continuing rehabilitation exercise
program.

I would appreciate it if you could provide a healthy food scheme with simple recipes as a guideline
and send to his home address, 9476 Old Dam Road,Goondiwindi,Qld,4390.Should you required any
further information regarding Mr.O’Riley please do not hesitate to contact me .
