Patient Teaching Plan

The patient has dementia and acute encephalopathy and is at risk for falls. The teaching plan aims to educate the patient on using the call bell and bed alarms to prevent falls and increase safety. Due to her medical conditions, the patient has limited cognition and is a visual learner. Teaching methods will include one-on-one discussion, demonstration, and visual aids. The goals are for the patient to state the purposes of the call bell, identify ways to prevent falls, and demonstrate call bell use, though frequent re-teaching is needed due to her conditions.
