AFFIDAVIT OF DISCREPANCY

I, Carina P. Barredo, of legal age, married, Filipino and a resident of P-4 Coro-Coro,
Tiwi, Albay, Philippines, having been duly sworn in accordance with law, hereby depose and
states:

1. That I am a duly Licensed Professional teacher;

2. That from the records of Professional Regulation Commission, Department of


Education and Civil Service Commission and other related offices, my birthdate
appear as November 7, 1970, however upon verification at Philippine Statistics
Authority, it was discovered that my true and correct birthdate is November 7, 1972.
Attached herein is a copy of my Birth Certificate as “Annex A”;

3. That I am executing this Affidavit of Discrepancy to attest to truthfulness of the


foregoing facts and to show my true and correct birthdate and for whatever legal
purposes this may serve.

Further, I am declaring that from this date hereon I would be using my true and correct
date of birth of November 7, 1972 in all my papers, records and other documents.

IN WITNESS WHEREOF, I have hereunto set my hand this 26th day of May 2017 at
Legazpi City, Philippines.
