SUBJECT: FIELD OBSERVATION REPORT

Undersigned visited MCHC Tarko on dated 30-4-2018 and found following observation

 Irrational use of injection syntocinon by Midwife and 7 injection used in one client
during labor named Hazoora w/o Ibraheem delivered on 23 April 2108 by MW
Samina evening shift She is not managed case properly and calling of MW dilshad for
helping( she is in morning shift ) Partograph shows fetal distress due to
mismanagement of case and this use of injection is verified from partograph and daily
expanse register it is very dangerous for mother and fetous and can cause serious
complication including fetal distress and maternal complication and in this case
partograph already shows fetal distress
 Another client Named RAHAT w/o Ashique Kubar came at health facility dated on 28
April 2018 in evening shift attend by MW Samina according to in charge MO she was
booked client but on verification from Maternal and Obs register there is no entry
during Antenatal care visit and during labour pain at Health facility client was not
delivered at HF and refer to MW Dilshad in private clinic for conducting delivery
MW Samina says that client is refuse for delivery by me and insist for me for calling
MW Dilshad for conducting delivery but MW Dilshad was refusing for coming at HF
for Delivary so that’s why client goes to own private clinic for delivery. Dilshad
accept that in front of in charge MO, both MWs and client attendant for conducting
NVD at private clinic and charge fee rupees 2000 This type of practice is not
acceptable and against in PPHI policies
 Partograph not filled as per protocol in all shifts
 Maternal, Obs, and family plaining register not maintained and updated as per
protocol
 Emergency Medicine box is not maintained as per protocol
 Neonatally Ambu bag with Mask is not supplied
 HBB kit and baby Towel sheet is not sterilized

Recommendation
 Appointment of Female Medical Officer on priority basis as soon as possible
 Action taken against Midwives Samina and Dilshad for Male practice at HFs
 Increases Monitoring visits in early morning and late night for regularity and
punctuality.
