Offer of Testimony

The testimony of the witness id being offered to prove:

1. That she is a witness of the incident that transpired on October 29,


2017

2. That she will also testify on the facts and circumstances


surrounding the case

3. She will also corroborate the testimony of the accused.

4. That she will identify the affidavit in relation to this case she
executed which will serve as her direct testimony

affidavit that you are referring to?

 The affidavit is written in English language, before you affixed your


signature, was this explained to you in a language you speak and
understand?

 Do you confirm that before you affixed your signature, you understand
the contents of your affidavit?

 Do you affirm and confirm the truthfulness of the contents of your


affidavit?

 Do you affirm that everything that is stated there in your affidavit is the
true narration of what transpired between the complainant and the
accused on ________?
