2. That I have recommended him to take TM 1 for we have already agreed to one condition.

3. That he refuses to accept my assigned loads to teach here in our Institution.

4. Despite all the recommendation, certification that we issued for him to take the said training,
he refuses to do so.

5. That I am executing this affidavit to attest to the truth of the foregoing facts and to request for
for the hold of issuance of his NTTC for him not to get it from the office of Tesda Provincial
Office.
