AFFIDAVIT OF LOSS

I, RIZA P. ESPIDA, of legal age, Filipino, and resident of #11 Trinidad


St., San Isidro Subd., Pamplona 1, Las Pinas City, after having been duly
sworn to in accordance with law, hereby depose and say that:

1. I am a holder of a claim stub issued by TESDA for claim of result


of the National Assessment NCII.

2. However, said claim stub was inadvertently lost.

4. Despite of diligent efforts to look for said claim stub, all such efforts
proved futile and in vain, thus, I am declaring the loss of the said claim
stub

5. I am executing this affidavit to attest to the truth of the foregoing


facts for all its legal intents, effects, consequences and purposes.
