Residences, Sampaloc, Manila, has consulted me with symptoms of fever for two (2) days

associated with cough and throat pain.

Diagnosis: Upper Respiratory Tract Infection; Acute Tonsilopharyngitis, Exudative

Remarks: Patient is advised to take prescribed antibiotics and to have a bed rest for 2-3 days

until full recovery.
