AFFIDAVIT OF UNDERTAKING

I, Ginang Siphayo, of legal age, single, Filipino with residence at 123


Zone VI, Guindapunan, Palo, Leyte after having been sworn in accordance
with the law hereby depose and state that:

1. I am the mother and co-maker of a Mitsubishi Mirage car loan


executed by my deceased son Juan Siphayo from Eastwest Bank with
plate no. NP 9716 which figured in an accident;

2. Sometime last JUNE 30, 2018 approximately Three o’çlock in the


early morning, he was a party to an accident whereby he was shot in
the head causing his immediate demise by the other party to the
incident who happened to be a police officer by the name of PO2
Mamang Pulis;

3. Because of such incident, the vehicle that he was driving was


impounded by the authorities;

4. I hereby undertake to surrender said car back to Eastwest Bank to


free myself from any obligation to pay owing the bank due to my
financial incapacity;

5. This affidavit is being executed to support my request for the release


of the impounded vehicle to Eastwest Bank;

6. I am executing this undertaking to attest to the truth of the


foregoing.
