Job Description: Surgical Ward


1. The staff nurse recognizes the nursing care plan
A. Organizes the diagnosis, the medical treatments,
the nursing activities so that the patient gets
maximum benefit.
B. Assesses patient’s response to illness, medical
and nursing care.
C. Reports favorable and unfavorable changes in
the patient’s condition.
D. Documents patients progress.
E. Provides encouragement, support and instruction
to a patient.
F. Head to toe assessment.
2. Implements nursing interventions (Nursing Orders)
Evaluates and revises nursing interventions

A surgical staff nurse plays a key part in the pre and post-
operative care.

PRE-OPERATIVE CARE
a. Observes and reports unusual information that
might influence surgery.
b. Assists the patient to identify and cope with
common concerns related to surgery.
c. Teaches patient what to expect and what is
expected before, during and after surgery.
d. Carries out pre-operative investigations and
preparations for surgery.
e. Coordinates pre-operative activities with
members of the health care team.

PRE-OPERATIVE NURSING ASSESSMENT

Head to toe assessment.

Documents any allergy.
Identifies, assesses and helps with patient’s concern about
surgery.
Checks if consent for surgery is accomplished.

IMMEDIATE POST-OPERATIVE NURSING CARE

The nurse collects information about the surgical


procedure to anticipate and to prevent complications
and plan care; type of anesthesia, duration of surgery,
the presence and location of drains and catheters, drugs
IV fluids or blood transfusion. Output and estimated
blood loss during surgery, difficulties during surgery
and underlying condition e.g. Hypertension, DM, Heart
disease, also the physician post-operative orders.

The nurse checks the vital signs as per protocol e.g. every
15 minutes for the first hour, every 30 minutes for the first
2 hours, then hourly, if stable every hours.

If surgery is done under GA, ambulate the patient the


same evening if there is no contraindications.

If surgery is done under spinal anesthesia, put the


patient flat on bed for 24 hours. Observe for spinal
headache and hypotension. Check for return of motion
and sensation at the lower extremities, side rails should
always be up.

Job Description: General Ward


1. Performs basic nursing procedures such as admitting
patients, and charting in the nurses notes. Bed making,
giving early morning care, evening and cleansing bed
bath. Oral hygiene, care of the back, prevention of
bedsores, feeding of patients with NGT, catheterization,
collecting laboratory specimen, administration of oxygen
and cleansing enema.
2. Carries out doctor’s order in accordance with
hospital policies and procedures. Administers patient’s
medications with care and notes reactions. Reports
adverse or abnormal reaction to attending physician or
surgeon.
3. Joins with the surgeon during rounds and carries out
doctor’s order using the kardex.
4. Prepares patients for surgery and patients who will
undergo special procedures like IVP, Ultrasound, X-
ray.
5. Administers pre-op medications and notify OR
personnel.
6. Takes care of patient post-operatively.
7. Monitors and records vital signs, intake and output and
refer if abnormal findings are detected.
8. Sets up treatment trays, prepares instruments and other
equipments. Assists surgeon in dressing of wounds, in
inserting NG tube and extubating patients.
