AFFIDAVIT
I, JAMES RAYAN MAINA, of post office Box 50924-00200, NAIROBI within the republic of
Kenya do hereby make Oaths and state as follows:-

1. THAT I am a male adult of sound mind and holder of Kenyan Identification Card
9825182 hence competent to swear this affidavit.

2. THAT I am the owner of the motor vehicle registered No. KAM 967U.

3. THAT the logbook of the aforementioned motor vehicle got lost and the matter was
reported at the police station.

4. THAT I request the Director and the Registrar of motor vehicles to issue me with a
duplicate logbook.

5. THAT I swear this affidavit conscientiously believing everything contained herein is


true and in accordance with Oaths and statutory declaration Act CAP 15 of the laws of
Kenya
