I, JUAN D. OLIVA, of legal age, married, and a resident of Hernaez Homes Subd., Brgy. Man,
Cebu City, Philippines, after having been duly sworn to in accordance with law hereby depose
and state that:

1. I am the sole proprietor of JDO Inc.,, a company duly organization under the
laws of the Philippines with principal office address at Mandaue Cebu.

2. I am applying for the closure of my business effective 23 January 2015.

3. The bookkeeper has erroneously processed the renewal last 05 January 2018
instead of the business closure.

4. I am executing this affidavit in order to attest to the truth of the foregoing, and
for whatever legal purpose it may serve.
