AFFIDAVIT OF LOSS

KNOW ALL MEN BY THESE PRESENTS:

I Angela Marquez, of legal age, Filipino, single, and a resident of 5313 18 St. Sto Niño Paranaque
City, after having been sworn in accordance with law, hereby deposes and states the following:

1. That I am a Senior High graduate of Manila Tytana Colleges last April 2018 with a student
number bearing 20166800128;

2. That a Form-138 was issued to me this year;

3. That sometime in June 2018 when I am currently preparing my requirements for my college, the
folder from which my documents are kept was lost;

4. That despite diligent effort to locate the said folder, I was not able to find the same; and

5. That I am executing this affidavit to attest to the truth of the foregoing and to request from the
Manila Tytana Colleges (Formerly Manila Doctors College) for the issuance of new certified true
copy of my F-138.

IN WITNESS WHEREOF, I have hereunto set my hand this ___ day of ________, 2018 in
Paranaque City.
