and faculty. Haley understands what it means to be professional and quickly
learned how to navigate office nuances – i.e. ability to be friends
with colleagues and knowing when it’s OK to joke around and when
more ‘professional’ approaches are needed.

Haley effectively saw the completion of a project that included

planning, implementation, and evaluation.

Instructor comment:
Haley demonstrates professional accountability, submitting
academic work, (poster, LEARN and Evaluation) in a timely manner
and of high caliber. The feedback from the evaluations and
qualitative comments demonstrate a team with a professional
approach, and ability to engage well with children in the school
while implementing Love your Life. She kept communication open

Preceptor comment:
Evidence informed practice was at the heart of this project and it is
obvious that her knowledge of such, coupled with her understanding
of the needs of the students (and teachers) resulted in a successful
implementation of the LML program.

Instructor comment:
Haley collaboratively with her peers, researched the needs of the
target populations and catchment area for the school. The team
used creativity, evidence and theories to inform Love my Life
activities, and evaluation methods. The outcome evaluations and
qualitative feedback were very positive.

context. Preceptor comment:
Haley was able to form meaningful relationships with everyone she
worked with - including PPH staff, and staff and students at St. Joe’s
alike. Collaboration was a key ingredient for this project to be
successful, and Haley was able to integrate many successful
elements into her project.

Nancy and I also want to highlight how well you did at the
presentation – actively listening and engaging the students @ St.
Joe’s and brining the presentation to life!!

Instructor comment:
Based on accounts above and collaborative work I have
witnessed in seminar, Haley is a strong team player, but is also
able to listen to other’s input and respond appropriately. She
understands the context and challenges of the students,
teachers and her colleagues, and is able to bring out strengths
and capacities of herself and others in their time together.

5.Demonstrated self-regulation Student comment: Engaged in self-regulation by reflecting on

allowed for appropriate planning and pacing.

Instructor comment:
Haley’s reflection showed insight into managing inappropriate
student behavior, learning student’s names, creating a
professional presence, and structuring activities to facilitate
engagement. Haley and her team mates were responsive to
the school’s needs, and facilitated the Love My Live program in
the allocated time with great results.

Student’s Comments:

Preceptor’s Comments: Nothing unsatisfactory at all – but this is the only spot to leave comments 
You were the backbone of this placement Haley and we so appreciate your effort and attitude. You offered a
fresh perspective on the program and we appreciated everything you did for us and accomplished here! Your
positive attitude and willingness to try new things outside of your comfort zone are going to take you places!
Thanks for everything!

Clinical Instructor’s comments: (Any area marked unsatisfactory needs to be commented on)
Haley has met or exceeded the expectations of the course objectives, tailoring Love My Life activities to the target audience of the elementary
students. She and her partner developed a theoretically based evaluation which demonstrated positive outcomes overall. She worked
collaboratively to be strong representatives of the School of Nursing and Peterborough Public Health. I agree with Haley’s strengths summarized
below. I anticipate the more she is immersed in chaotic situations, she will draw on this experience to pulling on her flexibility, resourcefulness
and strong interpersonal skills. This will support her in any clinical setting of her choice. Best wishes.
