I/We hereby willingly and voluntarily allow my/ our
son/daughter_________________________ to attend Saturday classes(January
16 & January 30,2016) as a make-up classes due to Pintaflores Festival
held last Nov.3 & 6,2015.
I/We have considered the benefits that my son/daughter will get from
his/her participation in this activity provided that due care and
precaution will be properly observed to ensure the comfort and safety of
my son/daughter and that DepED employees and personnel may not be held
responsible for any untoward incident that may happen beyond their
control.
