Medication Error Prevention

A medication error occurred when a nurse administered the wrong dose of a medication to a patient. The root causes were identified as lack of experience of the nurse, inadequate training, and failure to double check the medication against the prescription. A quality improvement plan was implemented which included additional training for nurses on proper medication administration procedures, a new policy to double check all medications, and regular competency assessments to prevent future medication errors.
