1. Edwin Llavore was a job order employee at the National Maritime Polytechnic Tacloban City who received a letter from the LTFRB-Iloilo City in 2018. 2. Due to an unintentional oversight, Edwin was unable to immediately give the letter to his supervisor, Romulo Bernardes, and it was left on his desk. 3. On December 3, 2018 when securing his office clearance, Edwin turned over all documents in his custody, including the letter, to Romulo Bernardes.

AFFIDAVIT

I, EDWIN LLAVORE, Filipino citizen, of legal age, single, and a resident


of ____________________, after having been duly sworn in accordance with law,
hereby depose and say, THAT:

1. I was a Job Order employee at National Maritime Polytechic Tacloban


City under the office of the Executive Director.

2. During my tenure at National Maritime Polytechnic, I received a letter


forwarded by the LTFRB-Iloilo City some time on 2018.

3. Due to completely unintentional and undeliberate chance, I was unable


to immediately give the said letter to Mr. Romulo Bernardes, and the
same was inadvertently left unattended on my table until I was in the
process of securing my office clearance.

4. On December 3, 2018, during the process of securing my office


clearance, I turned-over all the documents under my custody, and it
was only on that date that I was able to formally give the said letter to
Mr. Romulo Bernardes.

5. It was by a completely inadvertent chance that I failed to formally turn-


over the said letter to Mr. Romulo Bernardes, and I was completely
unaware of the importance of such documents.

6. That I am executing this Affidavit to attest to the truthfulness and


veracity of the foregoing statements and for whatever legal purpose it
may serve.

FURTHER AFFIANT SAYETH NONE.
