CASE OF EMMY

WHO IS EMMY?
• 19 years old
• Medical Technology student – 3rd Year College
• Metro Manila
REASON FOR REFERRAL:
• Depression
• 2 months prior – hard time sleeping and eating; feelings of loneliness and
hopelessness; irritable
• Day before admission – attempted suicide
• Currently, still reports feelings of loneliness and sadness from time to time
DECEMBER 2016
• Has a boyfriend of 3 years
• Month where she realized she is already
“obsessed” with her boyfriend
• Spends all her time with him
• Agreed to engage in intercourse
JANUARY 2017
• Pregnancy scare
• Stopped studies
• Became sales clerk (2 weeks)
• Cold treatment; frequent fights with bf
• SYMPTOMS:
• Occasional feelings of loneliness, there will be nights where she will just
cry and cry
FEBRUARY 2017
• Parents let her choose between staying with them or
choosing boyfriend
• Lived in boyfriend’s house for a month
• Boyfriend’s aunt did not like her (sinisiraan ako sa
boyfriend ko)
• Pregnancy test again – turns out she is not pregnant
• SYMPTOMS:
• Did not sleep well and often skipped meals
MARCH 2017
• Got jealous because she found out his BF
communicated with another girl (same day)
• Got hospitalized due to UTI/ulcer - parents
brought her to the hospital, boyfriend did not care
• Attempted suicide at their house using a small saw
BIOLOGICAL
• No history of mental disorder in the family
• No reported physical illness
• Not under any medication
PSYCHOLOGICAL
• Thinking patterns
• Taking responsibility for bad events
• She felt hopeless, she felt it was her fault that she became “pregnant” that’s
why she chose the bf over her parents
• Having inflexible rules about how one should behave
• Tendency to seek approval to boost self-esteem (parents, friends)
• Thinking others think badly of her (parents, friends and even bf)
• Sense of failure and low self-esteem
SOCIAL
• Parents – trust, contol
• Partner
• Friends
* little to no social support
THERAPY GOALS:
• To help patient cope up with her emotions more appropriately
• To aid in processing of relationship and family issues
• To help the patient understand and challenge her maladaptive thoughts of
self and others
• To increase activities that will improve her mood and relieve feelings of
anxiety
CLINICAL IMPRESSION:
• Emmy developed insight and capacity for self-reflection. She was able to
recognize her own issues.
• With regards to her relationship with her parents, particularly her father,
it still continues to affect her emotions. She manages mostly by avoiding it.
• Her rebellious phase might be tied up to her longing to be free and gain
control over herself.
• She can now assess her problems thoroughly and and come up with
different solutions before making a decision.
RECOMMENDATIONS:
• The parents can strengthen efforts of building a secure relationship with
Mae, for more trust to grow between them.
• Mae should also be encouraged to continue to participate and be active
in the different activities in the hospital. Once she is discharged, it is also
recommended that she continue to be active in her hobbies and
organizations, like dancing and playing sports.
• Mae is also encouraged to continue to monitor her mood by doing
weekly self-check. This way, Mae would increase self-awareness.
