 For SY 2019-2020, Grades 1-3 & Grades 7-9 will
adopt the updated SF10.
 SF10 will only bear the DepEd official logo and
seal.
 Any erasures or alterations on the document
should be validated and bear the signature of the
School Head in ink.

 SF10 shall be prepared at the end of the school


year or in the event of learner transfers within the
SY. No need to update every quarter.
 SF10 shall be printed using the ordinary legal
size bond paper, preferably in separate pages
and not back to back.
 Learners enrolled in Kindergarten shall use the
ECCD checklist instead of Form 137 or SF10.
